Job Summary In collaboration with a physician, diagnoses and treats certain acute or chronic health conditions in specific populations. Training is provided relevant to the population served, based on scope of care of the service assignment. Job Responsibilities Obtains patient history and perform assessments via observation, interview and examination. Orders, performs and interprets diagnostic studies. May be required to perform preventative health assessments, screenings, immunizations and care. Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources. Provides counseling and education of patients and their families/caregivers concerning preventative health, treatment options and community resources. Provides care in the context of promoting age and culture-appropriate informed choices, shared decision- making and the right to self-determination. Provides accurate and timely documentation of assessment and patient care plan. May coordinate the registration functions including collecting and entering demographic, insurance, employer information, and co-payments. Organizational Responsibilities Adheres to all organizational policies and procedures. Education Master's degree or higher in Nursing required. Licensure/Certification/Registration Licensed as an Advanced Practice Registered Nurse (APRN) or other designation as required by state law. National certification from one of the following American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners (AANP), American Association of Critical Care Nurses (AACCN), Pediatric Nursing Certification Board (PNCB) or National Certification for the Obstetric, Gynecologic, and Neonatal Nursing Specialties (NCCCOGN) required. Certified as a Basic Life Support (BLS) Healthcare Provider preferred. Work Experience No experience required. Experience in a specialty area preferred. Skills and Attributes Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action.
