Full time benefited position in our Reseda office location : 9am-6pm Thursday, Friday, Saturday, Sunday and Monday. Off on Tues/Wed. : LVN preferred but not required. Healthcare/hospice/home health experience preferred. : POSITION SUMMARY:: The Referral Coordinator is responsible for activities and actions related to promoting the services and expertise of hospice to the medical community and to patients and family members. QUALIFICATIONS:: * Bachelors degree or education in healthcare related field. Nurse or Social Worker preferred. * Certified, licensed or registered as appropriate. * Two years of health care experience required. One year experience in hospice preferred. * Knowledge of hospice requirements including Medicare, Medicaid, and private insurance guidelines. Good communication skills. Ability to respond to the needs of physicians, discharge planners, patients and family members. Ability to coordinate services with referral sources, hospice staff and patients and families. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:: * Addresses inquiries and requests for hospice information. * Responds to referrals for evaluation and/or admission to hospice program. * Verifies and completes all admission and financial information and conducts chart audits. * Assures there is a valid physician order for assessment and admission. * Confers with clinical management regarding admission decisions. * Coordinates admission process between all team members. * Explains the hospice program to patients, family members, and other referral sources in an accurate, complete and thorough manner. * Provides statistical information to the Administrator as requested. * Identifies key referral sources and promotes good relationships. * Assures efficient transition of patient unto hospice service, including but not limited coordination of equipment, supplies and transport. * Maintains continuing education through academic studies, seminars, and workshops. * Attends and participates in in-services, staff meetings and case conferences. * Completes all documentation in an accurate and legible manner. * Complies with hospice policies and procedures, laws and regulations and standards of practice. * Performs other tasks as may be assigned.
