PURPOSE OF THIS POSITION: The Hospice Transition Coordinator (HTC) is responsible for facilitating patient transitions of care and admission processes that includes coordination with a Team Director, Clinical Director and/or Director of Marketing. The HTC functions in the role of an RN for visit support. This includes providing nursing services to Care Initiatives Hospice patients, through visits that may include telephonic and in-person management of patients, families, caregivers and referral or community sources. The HTC establishes positive relationships with community sources with the intent to enhance and expedite patient referrals. The HTC establishes appropriate professional and public relationships that will encourage and develop short-term and long-term referral sources. QUALIFICATIONS : Education/Certification/License: * Bachelor’s degree in related field or equivalent work experience. * Registered Nurse education and training. * Current, valid Iowa Registered Nurse license. * Current, valid CPR certification * Obtains Hospice and Palliative Registered Nurse Certification (CHPN) within three (3) years of hire in this position. * Current secure home access to the Internet. Requirements: : * Must be 18 years of age or older. * Functional literacy in English. * Current driver’s license and automobile liability insurance. * Insurable by Care Initiatives auto insurance carrier. * Occasional travel by automobile with possible overnight stays, generally within the state of Iowa. Knowledge: * Knowledge of care of the terminally ill. * Knowledge of federal and state hospice regulations. * Understanding of the healthcare system. Experience: * At least 2 years as a hospice registered nurse or 5+ years as a registered nurse in clinical care. * Knowledgeable in transition of patient care services or continuity of care, preferred. * Experience in hospice care, post-acute care and/or admissions preferred. * Experience with assessing patients and administering medical charting and records. * Experience interacting with physicians and health care providers related to patient care. Skills/Abilities: * Ability to plan, organize and utilize teaching principles in working with families, patients and other community organizations. * Ability to exercise independent judgment and to independently work. * Ability to develop rapport with patients, families, medical providers, social workers, discharge planners, long-term care and assisted living administration, and community groups. * Ability to deal tactfully and effectively with the public ( community groups, etc.) and other employees. * Demonstrated tactful, supportive and sensitive approach to patient/family/caregiver needs. * Ability to effectively communicate both orally and in writing. * Ability to provide skilled nursing and palliative care to hospice patients * Ability to provide lifting, turning and ambulatory assistance to patients. * Ability to use the computer. Ability to use audiovisual equipment and software for presentation and teaching.
