The Licensed Practical Nurse (LPN) is responsible for providing direct patient care under the supervision of a registered nurse (RN). Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Provides direct patient care as defined by the Nurse Practice Act.
Implements plan of care initiated by the registered nurse.
Provides accurate and timely documentation consistent with the plan of care.
Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, and SLP) to the registered nurse and/or clinical supervisor.
Uses equipment and supplies effectively and efficiently.
Participates in personal and professional growth and development.
Participates in on-call duties as assigned

Supports a dignified and caring atmosphere with Home Health staff and field staff. All information obtained in the field, or at Home Health Agency, is kept strictly confidential.

Protects privacy and confidentiality of information pertaining to the Home Health Agency


Must have a current, unencumbered LPN license in the required state.
Must have CPR certification, and have a valid driver’s license.
Complies with accepted professional standards and appearance.
Excellent written and oral communication skills.

One (1) to Two (2) years nursing experience in community/home health is preferred.
LPN certification from an accredited practical nursing program or an Associate’s Degree in Nursing.
