Job Description

We are looking for a PRN Physical Therapist to work with our experienced team in our newly-remodeled space that includes a therapy pool.  This is primarily an outpatient position that treats a wide range of diagnoses with a 10% mix of inpatients.  New grads welcome.


Position Summary--The Physical Therapist is responsible for providing physical therapy services as determined by medical direction, therapist judgment, patient needs and desires, and available facilities. Organizes and conducts physical therapy programs in the hospital and community to restore function, relieve pain, and prevent disability following disease or injury. Helps patient reach his/her optimal physical abilities and learn to live within the limits of his/her capabilities.
  • Position Qualifications
    • EDUCATION:
      • Required:  Master's degree (MPT.) or Doctorate (DPT) in Physical Therapy from an accredited program.

    • LICENSES AND CERTIFICATIONS:
      • Required:  Active license from Iowa Board of Physical Therapy
        Basic Life Support (CPR)

·         Evaluates and develops appropriate care plans for outpatients and hospitalized patients of CCHS.  Coordinates care plan with patient, family and communicates findings and develops appropriate treatments to meet patient and family goals. ·         Consults and communicates with other clinical staff, providers, physicians and rehabilitation team to coordinate services rendered to patients regarding diagnosis and prognosis of patients to ensure safety and high quality care. ·         Utilizes electronic medical records to develop care plans, updated plans, daily notes and discharges in accordance with federal and state regulations.  ·         Develops appropriate treatment strategies and protocols to improve patient quality of life and improve overall wellbeing of patients. ·         Supervises treating Physical Therapy Assistants in compliance with state and federal guidelines.  Monitors progress of those patients and modify care plans when necessary. ·         Maintains and supports a positive, welcoming atmosphere for patients and co-works to give patients and families a superior experience. ·         Performs all other duties as assigned.
