Responsibilities: Educates and counsels patients and their family members after identifying their diabetic needs. Instructs patient on proper use of monitoring equipment per manufacturer's guidelines and hospital policies. Collaborates and communicates with other healthcare team members and the patient's medical provider in the delivery of care and education that meets the patient's needs. Audits patient's chart for accurate and complete documentation according to hospital policies. Collaborates and acts as a resource to other healthcare professionals with regards to diabetic education. Maintains quality assurance for diabetes education programs.
Requirements: Current license to practice as a registered nurse (RN) in the State of California. AHA Healthcare Provider or Red Cross Professional CPR Certification required. Previous experience in the field of diabetes education.
