Job Summary:

The LVN, under the supervision of the Medical Physician, provides nursing care to the assigned patient population.  The LVN will perform clinical responsibilities for pediatric, adult and geriatric patients and will work closely with the Medical Provider.  They are responsible for rooming patients, obtaining full vital signs, interfacing with other team members, facilitating care of patients and maintaining excellent electronic medical records in accordance with all policy and procedure guidelines.  The LVN will become proficient with nursing duties including preparing the patient for the examination/procedure, preparing and administering medications as ordered, Point of Care testing, instrument sterilization, wound care, documentation, EKG, assisting the Provider with examinations/procedures, facilitating prescription refills, facilitating patient referrals/transition of care, facilitating the scheduling of patient procedures, assisting the Provider with patient call back tasks and providing patient education/teaching.

 

Required Education/Qualifications:

  • Current California LVN License
  • Current American Heart Association CPR Certification
  • Basic Computer Skills
  • Excellent interpersonal and communication skills
  • Ability to work efficiently and with composure in a busy environment

 

Preferred Education/Qualifications:

  • Phlebotomy Certification
  • At least one year previous LVN experience

Job Summary:

The Licensed Vocational Nurse, working under the direction of a Registered Nurse, provides patient care and education following a plan of care established in collaboration with the patient, physician, caregiver and the nurse. The LVN functions as an active member of the home health team to achieve established goals. The LVN will primarily spend time throughout the day driving to clients' homes to provide care.

 

Required Education/Qualifications:

  • Current CA Licensed Vocational Nurse License
  • At least one year practical experience as a Licensed Vocational Nurse
  • Current CA drivers' license and auto insurance
  • Current American Heart Association CPR Certification
  • Ability to work and make decisions independently, while providing care congruent with the mission of Sonora Regional Medical Center

Preferred Education/Qualifications:

  • Home Health experience
  • Experience working with an elderly population
