Under the direction of the Clinical Manager and within established agency policy and procedure, the Physical Therapist provides a variety of skilled physical therapy services in the home care setting. Provides therapy to patients and families in their homes, which supports the total person. This is accomplished within an interdisciplinary plan of care. Works collaboratively with the Home Care Aide, Social Worker, RN, LPN, P.T.A., O.T., C.O.T.A., S.L.P., and office support staff to coordinate patient care utilizing community resources as needed. Participates in staff education. Provides consultation regarding assessment, planning and implementation of care. Coordinates activities with other community, agency and/or department staff and carries out special activities on assignment.
- Provides superior customer service to internal and external clients, customers and patients
- Exercises primary responsibility for a patient caseload, as assigned by Manager; functions as a case manager when necessary
- Provides assessments and customized treatment programs to patients in their homes, with the goal of improving mobility, restoring function, relieving pain, and preventing or limiting permanent disability.
- Responsible for reviewing all admission OASIS data and discussing with primary nurse if therapy is not primary
- Accurately, promptly and thoroughly documents patient care observations, interventions and evaluations
- Restores, maintains and promotes overall fitness and health
- Assumes accountability for own professional practice in achieving patient outcomes
- Attends and participates in multidisciplinary team meetings as scheduled by Clinical Manager
- Directs and aids patients in active and passive exercises; performs manual therapy techniques; performs therapeutic exercise, with and without equipment
- Provides guidance to family members and caregivers
- Orients PTA to patient POC need and monitors PTA progress in following plan of care to meet optimal patient outcomes
- Supervises and orients Home Care Aide to plan of care according to regulatory requirements
- Monitors patient progress to ensure accomplishment of patient goals and optimum patient outcomes; initiates changes as necessary and appropriate to role
- Completes OASIS timepoint assessments
- Collaborates with PI staff
- Successfully completes OASIS competency visit annually
- Participates in weekly case conferencing
- Collaborates and communicates with other health care providers to ensure quality and continuity of care
- Orients new patients to range of services provided by the agency
- Acts as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectively
- Ensures effective communication by checking voice mails twice per day and downloading PDA at least twice daily
- Collaborates with scheduling department to ensure patients are visited according to treatment plan, and fulfilling any productivity expectation
- Assumes accountability regarding communicating changes on patient schedules to Clinical Scheduler, and works with scheduling to maintain productivity levels if scheduled patient visits drop off
- Attends and participates in clinical inservices, trainings, etc.
- Adheres to the mission of the agency
- Current licensure from the Board of Allied Health in the State of Massachusetts
- Current valid driver's license
- Graduate of an accredited college of physical therapy
- Must have demonstrated competence in community health setting providing direct patient care with patients for a minimum of one - three years.
- Must be willing to work a minimum of one weekend every 6 weeks and two holidays per calendar year and as department may require.
