Collaborates with members of the health care team to provide medical nutrition therapy. Primarily focused on Clinical Nutrition for Inpatient/Acute Care and Transitional Care Unit/Rehab for patients of all ages.
Position Reports To:
Reports to Nutrition Therapy and Diabetes Manager.
Assesses nutrition needs and identifies nutrient requirements specific to disease state and age.
Continuously evaluates and monitors the effectiveness and outcomes of medical nutrition therapy intervention. Fosters continuity of care across the continuum of care.
Provides nutritional care to patients in assigned area in a timely fashion according to standard policies and procedures.
Provides nutrition education to patient and family.
Produces timely documentation in patients, Electronic Medical Record (EMR).
Work with interdisciplinary team members to provide quality nutrition care to patients.
Teach Cardiac and Pulmonary Rehab classes, community education classes, and staff education.
Participates in LEAN performance improvement and research activities.
May include outpatient nutrition counseling.
Required to maintain active Basic Life Support (CPR) certification and Washington State Food Handlers Permit upon hire.
Other duties as assigned.

O = Occasional, represents 1 to 25% or up to 30 minutes in a 2 hour workday.
F = Frequent, represents 26 to 50% or up to 1 hour of a 2 hour workday.
C = Continuous, represents 51% to 100% or up to 2 hours of a 2 hour workday.
