PRIMARY RESPONSIBILITIES:

* Monitor and document findings of newly admitted and readmitted patients and residents, individuals who have a change in skin integrity, a significant change in condition and/or as deemed necessary and according to center policy and procedure
* Evaluate wounds and other skin integrity concerns on a weekly basis, or more often as indicated, documenting findings, making recommendations for treatment, including pain, appearance, of wound edges and bed, peri wound, any signs and symptoms of infection, measurements, and other pertinent information. Document findings in accordance with center policy and procedure
* Complete scheduled treatments per physician’s order and document appropriately
* RN Treatment Nurses to stage wounds, develop and update plans of care
* Monitor and implement physician orders related to treatment of altered skin, ensuring orders contain required information
* Maintain knowledge about preventative skin measures, wounds, treatment options and interventions, and the like
* Submit weekly wound assessment and/or evaluation to Director of Nursing within determined time frame
* Manage and provide staff education on clinical devices associated with wound care, including negative pressure wound therapy
* Follow and oversee infection control measures related to altered skin integrity for both care and storage
* Advocate both preventative measures and appropriate treatment related to skin care and alterations to center staff, visitors and others as indicated
* Assist in staff education when necessary regarding wound prevention, identification, management of care and documentation
QUALIFICATIONS:

* Licensure as a Registered Nurse in WV
* Ability to communicate effectively both verbally and in writing
* Good time management skills
* Ability to work independently and with team
