Provide daily care coordination, case management, coaching, consultation and intervention to patients during care transitions and/or with one or more chronic diseases in order to promote self-management and preventive health care to drive better patient outcomes and proactively improve the their health.
The registered nurse will work as part of a care team within the practice to identify and outreach to Medicare patients and perform Medicare Annual Wellness Visits collaborating with the patient, provider and community resources to develop a personal preventive plan of care.
