Job Description

Responsible for interviewing all newly admitted patients to obtain the patients medication history including all prescription and non-prescription medications to reduce medication errors.

-          High school diploma or equivalent required

-          Registered in the state of Missouri as a Pharmacy Technician or Licensed Practical Nurse required, or ability to obtain

-          Completion of the Pharmacy Technician certification preferred  

-          Three years pharmacy work experience or experience taking medication histories preferred

-          Ability to take an effective medication history through excellent patient interview skills

-          Ability to utilize available resources to ascertain the accuracy of the medication list

-          Ability to work independently and in interdisciplinary terms and handle stress appropriately 

-          Ability to work cooperatively with other professionals and pharmacy staff

-          Ability to perform work accurately with attention to detail within specified time periods 

-          Ability to handle frequent interruptions and adapt to changes in workload and work schedule

-          Ability to set priorities, solve problems, and respond quickly to emergency requests 

-          Ability to communicate effectively (orally and in writing) 

-          Ability to utilize personal computer to complete word processing and computer order entry utilizing touch typing skills
