Under the supervision of the Director Specialty Programs or designee, perform all duties to support and in support of the VNA Maternal Child Health department to provide case management and direct nursing services to the MCH population. Communicate with patients and families in a manner that promotes goal achievement and exceptional customer service. Collect and record all relevant patient information. Assess health status changes, document and notify appropriate personnel. Perform other duties within the scope of position as assigned.Not applicable.A Bachelor's Degree in Nursing or the equivalent education and experience is required. Current RN Registration is required in both the Commonwealth of Massachusetts and in Rhode Island. BLS & CPR Certification is required. Hospital medical/surgical or other relevant nursing experience is preferred. Proficient computer skills, including experience with various software, database and electronic records programs is preferred. Two (2) years experience specializing in Maternal Child Health is required.
