Description:

American Health Network has taken the next step forward in healthcare in Population Health Management.  As part of our Population Health initiative, we are identifying high-risk patients and managing their care much more closely than ever before.  This includes assisting and building relationships with patients, meeting them during physician visits, educating patients on managing chronic diseases, and working with their physician to ensure their needs are being met.  The ideal candidate will possess excellent interpersonal skills, have the ability to provide flexible and creative problem solving techniques to alleviate barriers to care, and demonstrate the ability to work effectively as a member of an interdisciplinary team within AHN's primary care office settings in the Columbus, Ohio area. Ability to work at home at times. 

Duties:

  • Conducting comprehensive assessment of healthcare, educational, and psychosocial needs of the patient and their family.
  • Coach patients and caregivers in self-management
  • Enlisting a team approach by working collaboratively with the family, primary care provider and other members of the healthcare team to ensure coordination of services at multiple provider sites.
  • Facilitate access to community services.

Requirements:

  • Registered Nurse with an unrestricted and active Ohio license.
  • BSN preferred, but not required.
  • 2 years of clinical experience with preference for geriatric experience.
  • 3-5 years Case Management or Disease Management experience
  • CPR Certified
  • CCM (Certified Case Manager) certification preferred
  • Must be computer literate in EHR/EMR and Microsoft Office products
  • Must have an unrestricted driver's license.
  • Knowledge of state laws and regulations.
  • Experience with Medicare, Home Health, and/or Geriatrics a plus
