The Care Manager serves an integral role on an inter-disciplinary team whose purpose is to improve the ability of patients with chronic diseases to maintain and improve their health and avoid preventable hospitalizations and re-hospitalizations. The Care Manager will ensure continuity and coordination of care across transitions, including hospital to home; will identify barriers and facilitate optimal care delivery, including addressing medical, pharmaceutical and behavioral social needs; and will develop a comprehensive care plan to address those needs that include prevention, patient self-management, access to community resources and education about their chronic disease condition.
Minimum: Graduate of a Registered Nurse Program
Preferred: Bachelor Degree in Nursing
Current PA RN license, Act 33/73 Clearances, Healthcare Provider CPR, and valid driver's license and maintain eligibility to be insured under the Health System's motor vehicle liability insurance.
Minimum: One years clinical experience.
Preferred: One year psychiatric or chemical dependency nursing experience or charge nurse experience.
