Job Description

Position Objective:
The Emergency Department Advanced Practice Practitioner (APP) coordinates the health care needs of the ED patient population and provides appropriate direct patient care through assessment, diagnosis and treatment of specific conditions. Performs diversified clinical, educational, research and administrative duties in support of urgent and non-urgent patients, physicians and nursing and paramedic staff. Participates in the development and achievement of the philosophies, goals and missions of the ED and Frisbie Memorial Hospital. ?

Patient Population Served:
The Emergency Department APP is expected to demonstrate the knowledge and skills necessary to provide care based on the physical ability, intellectual development, psychosocial, education and safety needs of the neonatal, pediatric, adolescent, adult and geriatric patient. ?

Essential Position Functions:
Clinical: ?

Under the direction of the ED Medical Director and with established policies, procedures and practice guidelines of the hospital and Medical Staff, Standards of Care and Practice established by regulatory agencies and appropriate specialty organizations relevant to the care of the ED patient population, the incumbent performs the following functions: ?

Completes a thorough physical and psychological focused exam based on interview, examination and available data on neonates, children, adolescents, adults and elderly adults.
Gathers pertinent information from all sources: history, primary care provider, diagnostic test, patient and family, comprehensive physical examinations, behavioral and development assessments in order to establish appropriate priorities of care.
Orders and/or performs appropriate laboratory and/or other diagnostic testing.
Documents current illness/injury.
Documents physical findings, lab data, diagnosis and plan of care in the medical record.
Identifies medical and health issues and needs based upon the evaluation.
Formulates differential diagnosis based on history, physical and clinical findings and diagnostic testing.
Consults with the attending physician, patient's PCP and/or appropriate on-call consultants when unsure about any aspect of their patient's care or when necessary to plan appropriate follow up or admission.
Arranges/orders referrals and consultations with non-medical health care providers and arranges for medical consultation after discussion with primary provider.
Formulates, in collaboration with the patient/family physician, primary nurse and others, a plan that includes discharge and follow up care.
Performs routine diagnostic and therapeutic procedures according to established department protocols.
Writes and calls in prescriptions and behavioral responses to interventions for revision of management plan and documents changes in the medical progress notes.
Completes appropriate documentation for discharges and transfers.
Communicates with patient and family members regarding plan of care.
Identifies educational needs of patients/families and assists with teaching.
Address patient/family adaptations, coping skills and the need for crisis and other interventions.
Provides effective and compassionate care for patients and their families.
Ensures confidentiality of employee, legal client/patient relationships.
Performs procedures granted through the credentialing process.
Facilitates and/or coordinates patient care inter and intra departmentally at FMH and between FMH and outside agencies. ?

Administrative:
Participates in ED provider meetings for discussion of clinical and quality management practices.
Interfaces with management personnel to resolve problems affecting department operations and advises appropriate personnel of problems and recommends strategies for resolution.
Participates in self-evaluation to improve quality of care. ?

Non-Essential Position Functions:
Works collaboratively with and provides direction to the members of the multidisciplinary team.
Participates in continuing development activities, maintains professional affiliations and licenses and strives to increase medical skill and knowledge.
Promotes effective communication and positive public relations with clinical areas of responsibility.
Assists in developing, revising and implementing unit policies, procedures and protocols for the ED patient.
Serves on hospital/organizational committees.
Participates in the development of new programs for the ED.
Participates in quality management programs. ?

Position Qualifications:
Licensed to practice medicine in the State of New Hampshire.
Member of Frisbie Memorial Hospital Medical Staff.
Effective verbal and written communications skills.
Computer skills including keyboarding and mouse use.?
