VNA Community Healthcare and Hospice is growing and searching for Hospice Nurse (RN) case managers. This is a unique opportunity to join a new and growing hospice program. What You Will Do:
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Assesses and evaluates patients status by:
    • Developing and implementing an individualized, interdisciplinary patient family plan of care
    • Regularly re-evaluating patient and family/caregiver needs
    • Participating in revising the plan of care as necessary
  • Completes and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains /receives physicians’ orders as required.
  • Attends interdisciplinary group meetings.
  • Participates in on-call duties as defined by the on-call policy.
  • Attends six hours of in-service education related to hospice.
What You Will Need:
  • A qualified candidate for this position will have a valid CT Nursing License and one to three years of clinical experience in Hospice nursing or home health care.
  • Computer literacy with experience in clinical software systems and Microsoft Office Suite.
  • Driver's License and reliable automobile
