The Perinatal Nurse Navigator is responsible for outreach and coordination of education to providers of care for pregnant women who may be at risk of exposing their unborn fetus to addictive substances.
The Nurse Navigator will coordinate and collaborate with the Perinatal Center of Excellence team of physicians, perinatal intensive case managers, and perinatal certified recovery specialists to provide supportive treatment and medical services for pregnant women.
The nurse will facilitate interaction with primary care and specialists as needed for individual clients by direct communication or serve as a resource for case management staff in their collaborative efforts.
Nurse will provide in-home education to program participants on post-natal topics.
Nurse performs duties in support of the Medical Center mission to ensure the highest quality of client care in an economically sound and efficient manner. Performs duties in support of the Perinatal Centers of Excellence Grant to provide access to services, smooth transitions in treatment and be part of the community-based care treatment team

Education and Training:
Experience and Knowledge:
1 year experience Drug and Alcohol
Two years’ recent experience in maternal/child health, public health, home visiting, substance abuse or mental/behavioral health nursing preferred
Home visiting experience preferred
Adult education techniques
Registration, Licensure and/or Certification:
Current RN license in the State of Pennsylvania required
Certification in Mental Health Nursing preferred
Current child abuse clearance required
Valid Drivers’ License required and insured automobile required
CPR certification required
Excellent written and verbal communication skills
Computer Skills including data entry and Microsoft applications, word, excel

General nursing skills in assessment, planning, implementing and evaluating care
