Job Description

Position Purpose:  The Clinical Pharmacist is responsible for providing clinical pharmacy services and support to SHCN patients and providers. This position assists patients in various care management and clinical programs by optimizing pharmaceutical care. The Clinical Pharmacist also supports SHCN providers' efforts at meeting network incentives around clinical quality and medication utilization. The Clinical Pharmacist will also participate in clinical, operational, and health plan components as necessary to support SHCN objectives.

KEY RESPONSIBILITIES:
Identifies and telephonically manages patients who have complex medication-related issues, while coordinating care with other members of the care management team and SHCN clinicians.

Works closely with members of the performance team to drive performance in quality measures which have a clinical/pharmacy focus.

Conducts provider detailing (1:1, telephonically, at pod meetings, etc.) on pharmacy utilization and recommendations as needed to support clinical or medication utilization initiatives.

Serves as the medication expert for patients, providers, and SHCN team members.

EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:

Education:   

PharmD from an ACPE-accredited school of pharmacy 

PGY1 residency with experience in ambulatory care, or equivalent experience in ambulatory care.

Years of Experience:  

Minimum of 2 years of related clinical experience working in an ambulatory or outpatient setting.

Work Related Experience:   • Able to interact professionally with an interdisciplinary team including clinicians and non-clinical staff. • Able to educate clinicians and non-clinicians on medication therapy via verbal and written means appropriate to the customer. • Must have superior written and verbal communication skills. • Able to manage medication therapy for patients and populations. • Able to analyze and evaluate medical literature.  Specialized Knowledge:   • Current and valid Massachusetts pharmacist license. • Preferred: Certification by the Board of Pharmacy Specialties in a relevant area of pharmacy practice (e.g. BCACP, BCGP, BCPP, BCPS). • Familiarity with commercial and government insurance programs and related medication formularies.
