Registered Nurse - 2 South Renal Transplant
This position will be scheduled a minimum of 24 hours per week in the 2 South Renal Transplant Unit at the Christ Hospital main campus located at 2139 Auburn Avenue, Cincinnati, Ohio 45219.
Every 3rd weekend and half of all holidays plus call according to unit guidelines
This 26-bed medical/surgical unit specializes in the care of patients with kidney disease, including kidney transplant. The growth in the number of transplants in recent years is a testimony to the expertise of the team of board certified nephrologists, transplant surgeons and an all-RN nursing staff. A continuum of care is provided for patients undergoing kidney transplant as they are supported through their unique living related, cadaveric or paired exchange donation transplant process. The nursing staff is specifically trained to perform skills unique to this patient population, including peritoneal dialysis through manual as well as cycler methods. The nursing staff actively participates in delivery of specialized care for patients who qualify for cutting-edge pharmacologic research studies under the supervision of nephrologists, transplant surgeons and clinical pharmacologists. An on-site outpatient kidney transplant clinic provides ongoing episodic care to transplant patients. The unit also provides care for a wide variety of general medical/surgical patients with common diagnoses. This mix of patients provides a dynamic environment where nurses can meet career goals.Job Overview:
This registered nurse uses critical care specialized knowledge, judgment, and skills to provide care to patients in the 2 South Renal Transplant Unit. The RN utilizes knowledge derived from the principles of biological, physical, behavioral, social, and nursing sciences to assess, plan, implement, and evaluate patient care. All care is provided based on the concepts inherent in the model of care for TCHHN (The Christ Hospital Health Network) which promotes an on-going partnership between patients and families and the team of healthcare providers. The RN provides care is culturally based and age specific. The RN adheres to the American Nursing Association (ANA) code of ethics and to the scope of practice described in the Ohio Nurse Practice Act.
The RN may perform the following duties:
Complete a holistic assessment of the patient’s physical, psychosocial, spiritual, and educational needs utilizing The Christ Hospital Professional Practice Model.
Utilizes collected data to establish and prioritize a list of actual, and potential, individualized patient problems and needs.
Prepares, coordinates, and revises multidisciplinary plan of care in collaboration with the patient and family to be supportive of the TCH model of care/Patient and Family Centered care. For each problem a measurable goal is set.
Communicates the plan of care utilizing SBAR and handoff processes with all team members.
Performs and delegates nursing actions based on appropriate orders and demonstrates critical thinking and accountability for appropriate implementation.
Provides ongoing, individualized holistic patient and family education based on needs and plan of care in collaboration with the multidisciplinary team.
Responds to emergency situations that require an immediate, controlled precision in a skillful manner.
Implements measures to prevent contamination and/or transmission of disease to include the use of appropriate protective devices and equipment to prevent injury and maintain a safe environment.
Evaluates effectiveness of nursing care interventions, adjusts or continues with plan of care based on patient and family response.
Documents information in a timely manner, according to hospital policy, to include the plan of care, accurate updates, teaching and the patients’ and family response to care.
Associates Degree in Nursing Minimum (BSN required within 5 years of hire)
6 months of RN experience required, 1 year of inpatient med/surg or telemetry RN experience is preferred
