The RN Case manages, coordinates and evaluates the plan of care with therapeutic, social service, pastoral care, home health aide services, other organizations and physicians for an assigned caseload.  The RN instructs patient or responsible caregiver in nursing care, disease management, health promotion as directed by the physician's plan of treatment and consistent with Agency regulatory and professional standards.
