Job Description


American Health Network's Population Health Ohio Team is seeking a RN Clinical Supervisor to support the needs of our Population Health Team in the Central Ohio area. This includes assistance to Ohio's Clinical Manager and the Population Health nurses with their care coordination/case management duties in their family practice settings. The candidate would be able to maintain a high level of confidentiality, professionalism, and interpersonal skills with a motivational leadership style that aligns with corporate goals and initiatives to engage staff.   • Assists team members in performing duties; provides support to staff.
  • Serves as a liaison between physicians, managers, and staff members.
  • Identifies opportunities for process/quality improvement in all aspects of Population Health and patient care.
  • Provides care coordination and transition of care services by covering for staff, if needed.
  • Attends meetings and training at corporate headquarters and other locations, as needed.
  • Makes recommendations to management.


  • Develops staff through coaching, mentoring, rewarding, training and guiding.
  • Assists in aligning people and projects to achieve initiatives.
  • Assists in interviewing, hiring and orienting direct reports.
  • Assesses current and future staffing needs based on organizational goals.
  • Assists in managing employee issues and resolving grievances.
  • Assists in completing performance evaluations for direct reports.
  • Assists all staff in the interpretation of policies and procedures.


  • Unrestricted Ohio State Nursing (RN) License.
  • Bachelor's degree preferred or Associate's degree in Nursing with equivalent experience.
  • At least 5 years of nursing experience preferred.
  • One to three years of Case Management / Care Coordination experience preferred.
  • One to three years of experience in a clinical supervisory role preferred.
  • Case Manager Certification (CCM) or (BC) ideal. 
  • CPR certification required.
  • Unrestricted Ohio State Driver's License.


  • Must have strong interpersonal and communication skills.
  • Must have the ability to multitask coupled with critical thinking and analytical skills.
  • Must understand State and Federal healthcare regulations regarding healthcare services as well as understand Pay for Performance and Population Health Initiatives.
  • Must have and maintain a current knowledge of evidence-based case management and disease management best practices, including care management protocols and care plans.
