This is a case management RN position for home health care.  Patients are seen in their home for nursing care.  The normal hours are 8-4:30pm.  There are some weekends and some evenings.  There can also be some on call but it varies.  This RN works with the patients to improve their overall health status and to gain independence.  We strive to keep our patients out of the hospital and manage their care at home.  SUMMARY OF THE JOB  Provides comprehensive nursing care to patients. Is responsible for the clinical supervision of Aides, Licensed Practical Nurses, and student nurses in the management of the patient's plan of care. These services include but are not limited to: a.            Establishment of health care goals and objectives to improve patient care outcome. b.            Recommendations for solutions for patient's needs including appropriate referrals to other disciplines or community resources. c.             Teaching and supervision of patients and support system in their nursing care. d.            Recognizing physical and emotional illnesses and/or disabilities and assisting patients in securing appropriate referrals. e.            Administering medications and treatments as prescribed by the patient's physician. Conforms to established Agency expectations of productivity. Identifies and reports significant conditions and symptoms to physicians and other health care members; updates physician's orders according to agency policy. Documents nursing care and other significant information in a timely fashion. Maintains a clinical record that meets the requirements of policy and procedure and accepted standard of practice for each patient receiving care. Is responsible for coordinating patient care, promoting continuity of patient care, and communicating with other departments and on-call supervisor as appropriate. Improvises and manipulates the patient's environment so that skilled nursing care can be adequately provided. Demonstrates awareness and support of the Agency’s outcome objectives. Participates in on-call, holiday coverage, weekend rotation as necessary to provide 24/7 nurse availability. Participates in Agency and community health activities as requested. Participates in case conferences as part of the interdisciplinary team. Assists in the orientation of new personnel and/or student nurses. Supervises the LPN and/or Aides by communicating and updating the plan of care. Attends meetings and/or workshops as requested. Be a part of the Food & Nutrition Department by working in the newly remodeled Midtown Cafe. Staff working in this area will be trained and cross-trained in the various stations in the cafe, the C-store, and in the Doctors Lounge. Many of these positions require staff to prepare food items in front of the customers as well as in the back of the house preparing food for other stations. Staff also will learn how to use the register, work on the dish machine, and stocking. Our staff members have constant interaction with patient family members, staff from other departments, and other guests which requires our staff to have great customer service skills. ServSafe Certification is recommended, but not required.

Be a part of the Food & Nutrition Department by working in the newly remodeled Midtown Cafe. Staff working in this area will be trained and cross-trained in the various stations in the cafe, the C-store, and in the Doctors Lounge. Many of these positions require staff to prepare food items in front of the customers as well as in the back of the house preparing food for other stations. Staff also will learn how to use the register, work on the dish machine, and stocking. Our staff members have constant interaction with patient family members, staff from other departments, and other guests which requires our staff to have great customer service skills.
