Summary of the Job To coordinate and provide social services case management in accordance with acceptable social work practices and Deaconess Family Medicine Residency (DFMR) policies and procedures.  Works in close collaboration with members of the health care team to provide services and optimize patient functioning in the most efficient and effective manner to improve health outcomes. Implements grant-funded educational initiatives and services for targeted patient populations (e.g., prenatal patients, etc.) Document each patient or family contact in chart within 24 hours of encounter. Develop case plans in conjunction with medical provider for each patient who requires long-term follow up (more than three (3) visits). Record social history in EMR for patients who require long-term follow up. Follow up on care plan and referrals with patient. Maintain knowledge of relevant community services. Act as liaison between these agencies and DFMR. Shares resources with other DFMR case managers. Coordinate care and maintain communication with medical providers. Participate in multidisciplinary case conferences as needed. Report discovered abuse (child or elder) abuse to appropriate authorities/governing bodies. Demonstrate awareness of legal issues in all aspects of care coordination, governmental reporting and referrals and adheres to ethical standards of profession.. (ex. COBRA, EMTALA) Maintains client confidentiality.   Assists with special projects as assigned.

The Instrument Technician II sterilizes all instruments, equipment, linen and supplies using various types of autoclaves.  Loads autoclaves in the prescribed manner and sets controls to specified time and temperature according to manufacturer’s recommendations.  Completes appropriate documentation records prior to sterilization.  Reads and initials autoclave graph to verify appropriate sterilization cycle and provides required records for inspection.             Assembles all instrument procedure trays according to the prescribed manner using content lists and IFU’s as guides.  Performs a double check on all cannulated instruments and all consignment items.  Wraps packages and handles all items following procedure.  Marks items with identifying data and distributes or stores in designated area.  Notifies Team Leader/Manager when unable to replace parts or when equipment is deficient in the work area.   Disassembles and cleans instruments using approved disinfectants, detergents, soaps and cleaners.  Reassembles instruments after cleaning and test for proper functioning.  Places special focus on cannulated items by forcing air, forcing water/alcohol and running a brush through all cannulations.  Replaces lost or damaged instruments from stock on hand.    Completes required forms and transfers inoperable instruments to the appropriate holding area for repair and notifies appropriate personnel in surgery of malfunctions.   Dispenses products requested for sterile and non-sterile equipment and supplies.  Assigns priority to emergency requests and issues required supplies and equipment based on knowledge of procedure intended.  Assembles and wraps instruments according to Manufacturer’s Instructions for Use (IFU) while establishing aseptic technique.   Maintains assigned work areas and equipment in a clean and organized condition to maintain required standards for handling sterilized and clean materials and to maintain a safe work environment.   Performs other related duties as required, such as examining expiration dates, reporting inoperable equipment, making visual inventory inspections to maintain adequate stock and supply levels, tests effectiveness of autoclave function by performing Bowie Dick tests, culture spore tests (BI) and leak tests, stores equipment and supplies, cleans shelves and work areas and mops liquid spills from floors as necessary.   Delivers supplies and pulls cases for OR.  Answer calls from the OR.  Checks the integrity and all indicators, like color change on tape, on a sterile package(s).   Works with Center Core Technologists to coordinate special instrument requirements for surgical use.   Works with outside vendors on consignment instruments for surgical procedures.   Participates in Quality Improvement activities related to ancillary services.   Any instrument technologist will be on the Three Strike Disciplinary rule in SSP.  This is explained in SSP Policy and Procedures. Post Anesthesia Care Unit Our Post Anesthesia Care Unit (PACU) participates in the pre-operative preparation of surgery patients and provides immediate post-anesthesia care for all surgical patients (including ICU, Pediatrics, Same Day and Admission patients).    SUMMARY OF THE JOB The Registered Nurse provides direct patient care; demonstrates the knowledge, critical thinking and judgement of and integrates the use of evidence-based outcomes for Nursing practice; supervises care delivered; coordinates plan of care; provides education for patients, families and staff, and collaborates with other disciplines.   EDUCATION AND EXPERIENCE                      Graduate from a nationally accredited school of professional nursing, no experience required. If graduate of non-nationally accredited school of professional nursing, then at least one year of professional nursing experience is required.
