Develops a plan of care for each patient upon order or referral of a physician, setting short and long term goals and includes a discharge summary. Maintains adequate records and progress notes on each patient and completes daily billing. Participates in patient care conferences with other related health services, patient, and family to review patient’s progress and evaluate goals. Plans home instruction for patient, family, friend, or public nursing services. Delegates selected forms of treatment to supportive personnel with assumption of responsibility for care of patient and continuing supervision of supportive personnel. Maintains awareness of services that Medicare or other insurance will cover for Physical Therapy services and consults with patient, physician, family, or legal guardian regarding expenses. Works with other Medical Center personnel to set up programs to prevent potential disability and acquaints them with the objectives of Physical Therapy. Assists in setting up department policy. Assists in setting up programs to better serve the health needs of the area. Demonstrates resourcefulness in emergency situations. POSITION SUMMARY A Pain RN Assistant is a Registered Nurse (RN) who assists in the care continuum of pain patients, ensuring exceptional clinical-communication between the Clinic, Surgery and Nursing areas. The Pain RN Assistant shall be considered part of the clinic nursing team assisting in the rooming of clinic patients, assessing and managing needs and symptoms and effective EMR documentation and providing patient education in support to patients and their families as a means of continuity of care for all pain patients. The Pain RN Assistant will support both the clinic and surgery department schedules and needs on this campus. POSITION SUMMARY Under the direction of the charge nurse or patient nurse, the Critical Care Unit Assistant assumes responsiblity to carry out and coordinate communication at the nurses' station, monitor telemetry and EKG recording/interpretation and to communicate to the patient's nurse or charge nurse when there is a significant change or sustained pattern of change that is outside of the baseline for any telemetry monitored patient.
